Convenient, Minimally Invasive Care in a Comfortable Office Setting
At Midland Surgical Associates, many diagnostic and treatment procedures can be safely performed in our office without the need for hospital admission. These minimally invasive, outpatient procedures are designed to reduce recovery time while maintaining high standards of medical care.
Our in-office services allow patients to receive timely treatment in a familiar, comfortable environment under the care of experienced surgeons.
MammoSite® Breast Brachytherapy
Following a lumpectomy for breast cancer, some patients may be candidates for MammoSite®, a form of accelerated partial breast radiation therapy that significantly shortens the duration of treatment.
During this procedure:

A small balloon catheter is placed into the lumpectomy cavity through a small incision

The balloon is inflated with saline and remains in place for approximately five days
-
Radiation is delivered directly to the surrounding tissue using a high-dose-rate (HDR) machine
-
The catheter is removed after the treatment course is completed
This targeted approach limits radiation exposure to healthy tissue and allows patients to complete radiation therapy in a shorter timeframe.
Mammotome® Breast Biopsy
The Mammotome® device is used to perform minimally invasive breast biopsies in the office under local anesthesia and ultrasound guidance.

Tissue samples are sent to pathology to determine whether a mass is benign or cancerous
This procedure provides accurate diagnostic information while minimizing disruption to daily activities.
Skin Procedures
Benign Skin Lesion Removal
Many benign skin conditions, including lumps and noncancerous tumors, can be treated through in-office excision.

Wide excision and minimal excision techniques are available

Procedures are quick and performed under local anesthesia

Minimal dressing is required, typically for about one week
Shaves and Biopsies
For certain skin growths, shave excisions or curettage biopsies may be performed to aid diagnosis and treatment.

A superficial layer of skin is removed using a scalpel or curette

Specimens are sent to pathology for evaluation

Healing typically occurs within one to two weeks

Rubber Band Ligation
Rubber band ligation is a common in-office treatment for second-degree internal hemorrhoids.

A small band is placed at the base of the hemorrhoid

Blood supply is cut off, causing the hemorrhoid to shrink and fall away
-
Less pain and quicker recovery compared to surgical hemorrhoidectomy
Vein Treatments
Sclerotherapy
Sclerotherapy is a minimally invasive treatment for spider veins, particularly on the legs.

A sclerosing solution is injected into the vein

The vein collapses and is gradually absorbed by the body

Compression stockings or bandages are worn for several weeks
Laser Vein Procedures
Midland Surgical Associates uses the Dornier MedTech 940 Laser, a state-of-the-art system for cosmetic spider vein removal.

Effective for red and blue spider veins on the face and legs

Performed in approximately 25-minute sessions

Typically requires two to three treatment sessions

Treatments are spaced about four weeks apart for optimal results
Because spider vein treatment is generally considered cosmetic, these procedures are not typically covered by insurance.
Understanding Spider Veins
Spider veins, also known as telangiectasias, are small red or blue veins that appear just beneath the skin’s surface. Unlike larger varicose veins, spider veins are smooth and do not bulge.
They commonly appear:

While often cosmetic, spider veins can sometimes be associated with underlying venous insufficiency. Symptoms may include:

Throbbing

Occasional bleeding
In some cases, an ultrasound may be recommended to rule out deeper venous disease.

Spider Vein Treatment Options
Laser Therapy
Laser energy targets the hemoglobin in the blood, causing the vein to collapse and fade over time. This does not interfere with normal circulation.

Sclerotherapy
Best suited for certain leg veins, sclerotherapy involves injecting a solution that causes the vein to close and disappear over time. Advanced vein lighting technology allows for identification of feeder veins, improving outcomes and reducing recurrence.

Recovery After Vein Treatment
Laser and sclerotherapy treatments are outpatient procedures, allowing patients to return to normal activities the same day.

Debridement
Debridement involves removing damaged or infected tissue to promote healing of healthy surrounding tissue.
